First Visit
First Visit will consist of a 30 minute interview between Pharmacist and Primary Caregiver(s) Follow-up to be done 30 days after initial visit Copy of Medication Profile will be provided to Primary Caregivers(s) Our Club Members will be able to access our Pharmacist for any Emergencies 24/7
